• A letter from the institution from which you have graduated in Lebanon stating that you are under no financial obligation to that institution OR a copy of your degree from that institution.
• Case file number issued by the US Department of State after completing the online application available on the J Visa Waiver webpage: https://j1visawaiverrecommendation.state.gov/
• A copy of your passport, Visa, and I-94.
• A notarized statement from you stating that you are under no financial obligation to any private or public institution in Lebanon.
• Exact date of arrival on the J-1 Visa and the port of entry. Your address of residence while in the United States (if there have been any changes in your address, please list them all).
• Statement from you explaining whether your education in the United States is governmentally or privately financed.
• $125 money order payable to the Embassy of Lebanon.
• 2 addressed stamped envelopes: One addressed to the US Department of State* (see below for address) and one addressed to you.

• A letter from the applicant stating the purpose, destination, and date of trip
• A valid Lebanese passport
• Money order in the amount of $125 payable to the Embassy of Lebanon

The necessary papers required to obtain a letter from this Embassy for an extension of stay in the United States ( J-1 visa).

• A notarized statement from you confirming to the Embassy that you will return to Lebanon upon completion of training in the filed of _ _ _ _ (you must indicate your field of specialization) in the United States,and intend to practice medicine in the speciality for which training is being sought.
• A copy of the first 3 pages of your passport, a copy of your I-94 and the visa.
• Letter from the Institution stating that you are enrolled in a training program or a copy of the acceptance letter in the program.
• A copy of your Diploma
• $125 money order payable to the Embassy of Lebanon.

Ikhraj Qayd إخراج قيد

Prior to starting a new Ikhraj Qayd (إخراج قيد )
request, Applicants are required to check the Jurisdiction guidelines in
order to verify whether their state of residence falls under the Lebanese
Embassy’s jurisdiction (Washington D.C), or under its affiliated Consulates
General ( Los Angeles, Detroit or New York).  

Note: This request is directed towards applicants in pursuit
of the civil bulletin form (إخراج قيد)
ONLY, not the Lebanese ID card (بطاقة الهوية). Note:
 the request for a Lebanese ID card must
be fulfilled in Lebanon as the Embassy of Lebanon in Washington D.C does not
process them. Furthermore, clients are encouraged to apply for the civil
bulletin form (إخراج قيد)
directly from Lebanon as well due to their quick turnaround.

 In order to process a new Ikhraj Qyad request, applicants
are requested to send an email to consular@lebanonembassyus.org
with a copy of an old family/individual ikhraj kayd or Lebanese identification
card and a copy of the passport if available. Address in the U.S and Phone
number are required.

 N:B: Please note that it will take up to 3 Months for the request
to be completed. Once the embassy receives the Ikhraj Qayd from Lebanon, clients
are requested to mail in one large empty, stamped, self-addressed envelope from
USPS or Fedex along with a $20 money order/cashier check payable to the embassy
of Lebanon covering the translation of the Ikhraj Qayd if needed. Clients are
advised to keep a copy of their return envelopes, as the embassy is not
responsible for any lost return mail (in person applicants are exempt from this
step).

applicants are requested to send an email to consular@lebanonembassyus.org with a
copy of an old family/individual ikhraj kayd or Lebanese identification card
and a copy of the passport if available. Address in the U.S and Phone number
are required.

Criminal Record Report

Prior to starting a new Criminal record  request, Applicants are required to check the Jurisdiction guidelines in order to verify whether their state of residence falls under the Lebanese Embassy’s jurisdiction (Washington D.C), or under its affiliated Consulates General ( Los Angeles, Detroit or New York ). 

Requirements for Lebanese Citizens:

1- Copy of applicant’s إخراج قيد الفردي أو العائلي  (recent family/individual civil status bulletin)  or بطاقة ألهوية  (Lebanese identification card) and a copy of the passport if available.

2- Complete address in US along with Phone number

3- A money order payable to: “The embassy of Lebanon” for the amount of $60 

4 — A separate money order payable to: “The Embassy of Lebanon” for the amount of $80 for the translation of the criminal record

Requirements for Non-Lebanese Citizens:

1- Copy of applicant’s passport along with stamps showing entrance and departure from Beirut Airport.

2- Complete address in the USA along with phone number

3 – Dates spent in Lebanon and residing address

4- A money order payable to: “The Embassy of Lebanon” for the amount of $60

5 – A separate money order payable to: “The Embassy of Lebanon” for the amount of $80 for the translation of the criminal record

1-Wills, Affidavit of Heirship, or any documents related to inheritance, should be registered first at the probate court and then authenticated by the Secretary of State, and the State Department in Washington, D.C., before mailing them to the Embassy of Lebanon.

2-Failing to fulfill the above-mentioned requirement prevents the Embassy of Lebanon from processing any inheritance-related document.

3-The Embassy of Lebanon can only authenticate the will after the death of the testator.

4- Requirements:

• The Original death certificate;
• Copy of the applicant’s ID, and contacts (phone number and email address);
• Final will or affidavit of heirship registered at the probate court and authenticated by the Secretary of state;
